Targeted Case Management (TCM) at MCH

Targeted Case Management (TCM) at Maine Children’s Home (MCH) is a supportive service that helps youth and their families who are dealing with behavioral health issues, intellectual disabilities, or significant medical conditions. Through this service, a Case Manager works closely with families to assess their unique strengths and needs. They help create a personalized care plan and guide families in accessing valuable resources and treatment options. Additionally, the Case Manager tracks progress and provides follow-up to ensure that the services are effectively addressing the child or teen’s needs.

A Targeted Case Manager can help with:

  • Assessing the strengths and needs of the child and family regarding medical, educational, social, or other services
  • Helping to develop a plan to meet those identified needs
  • Providing education to families and children about available services and systems
  • Finding a range of suitable and accessible resources
  • Identifying treatment options and provide referrals to relevant services
  • Coordinating and oversee the delivery of services
  • Scheduling appointments
  • Supporting family members with organizational tasks, such as managing paperwork and attending meetings (including Individualized Education Program (IEP) meetings) to advocate for the child's needs

Eligibility for Targeted Case Management

Those who are eligible for Targeted Case Management are children or youth who have a mental health diagnosis, intellectual disability, or developmental disorder. Youth must also be within the age range of infant through 21 and covered by MaineCare, or youth under age 19 with Katie Beckett waiver.

Hours and Insurance Accepted

Maine Children’s Home offers flexible hours for Targeted Case Management. MaineCare is required for participating children.
